What we assess (readiness domains)

• Understanding of procedure & informed consent

• Motivation and expectations

• Adherence to medical/nutrition guidance & follow-up

• Eating patterns (binge/emotional/graze), night eating

• Mental health history & current stability

• Substance use risk and recovery supports

• Suicide/safety risk and protective factors

• Social supports & practical barriers (transport, time off)

• Coping skills, stressors, and behavior change plan

Methods (evidence-based, right-sized)

• 60–75 min clinical interview (+ collateral if needed)

• Standardized screeners as indicated (e.g., PHQ-9, GAD-7, AUDIT-C, PCL-5, EAT-26,C-SSRS)

• Brief cognitive screen when clinically necessary

• Coordination with your team for program-specific forms

• Diagnostic impressions (DSM-5-TR)

• Readiness determination: Ready • Ready with supports • Postpone & refer

• Specific, actionable pre- and post-op recommendations (nutrition, therapy, groups, relapse-prevention)

• Behavioral plan aligned with your program’s protocols

• Documentation that meets typical insurer requirements; we’ll complete your program’s template if provided

Turnaround: delivered via secure fax or email within 3 business days.

Referral & workflow

1. Refer: Call, Fax, or email the referral or use our secure online form (include contact, insurance, and target surgery date).

2. Schedule: We offer the patient the next available telehealth slot; portal forms due 24 hours before the visit.

3. Evaluate: Interview + screeners; we contact you same-day for any safety concerns.

4. Report: You receive the full report in 3 business days, plus a one-page summary if preferred.

• Same-day risk alerts (suicide, substance relapse, acute instability)

• Adult candidates (18+). Adolescent evaluations are available by program request with guardian participation.
